letter from doc to daycare?

pHello everyone! /p
pMy 14 month old son will enter daycare (2-3 days a week) come mid-September. They require special instructions in writing from the child's doctor for issues such as PA./p
pI meet this coming Monday with asst. director and U.S.D.A. service supervisor. I'm anticipating their reluctance to have/use an epi-pen so I want the necessity of the epi-pen mentioned in the letter too. /p
pMy doc said it would be best if I wrote one and then let him review it and put it on letterhead and sign it./p
pSo...although I did a search here (and on the main discussion board) I can't seem to find any sample letters like the one I need. /p
pCould you direct me to a thread or post a sample letter here? My doc will sign it Monday morning before my meeting so I'm looking to type it up this weekend. /p
